The matrix, or Matrix of Leadership, is an oval shaped crystal that is passed down from leader to leader of the autobots.
Prime is a honorific attached to the name of the autobot posses the matrix.
Now, the first Prime differs from every generation, but the original goes from Prima > Prime Nova > Sentinel Prime > Optimus Prime > Rodimus Prime (only in cartoon version)
On the battle of Cybertron, Sentinel was about to die and passed the matrix to Optimus, thus making him Optimus Prime and the leader of the autobots. Also, Optimus is said to be a direct decedent to Prima, the first Prime, making him the next leader regardless of Sentinels choice.
